Everlywell - Nurse Practitioner (contract)
Requirements
• Valid Nurse Practitioner license in at least 15 states. • Master's degree in Nursing or related field. • Minimum of 2 years of experience as a Nurse Practitioner, 1+ years of Telehealth experience is not required but preferred. • Proficiency in conducting physical examinations and interpreting diagnostic tests. • Strong knowledge of anatomy, physiology, and pathophysiology. • Excellent communication skills, both verbal and written. • Strong critical thinking and problem-solving abilities. • Medicare enrolled strongly preferred. • Please note this is a contract position
Responsibilities
• Provide comprehensive care to patients in a remote setting as part of the Remote Care Services team. • Conduct patient assessments and develop treatment plans based on individual needs. • Communicate effectively with other healthcare professionals regarding patient cases, ensuring continuity of care across different providers if necessary. • Maintain accurate records for each patient encounter to ensure proper documentation in the medical record system used by Everlywell. • Utilize telehealth technologies and platforms as required to deliver services remotely. • Engage with patients through various communication methods, including phone calls, video conferencing, or secure messaging systems provided by Everlywell's platform. • Monitor patient progress over time and adjust treatment plans accordingly based on feedback and clinical outcomes. • Collaborate closely with the broader healthcare team to provide holistic care for patients in a remote environment. • Attend virtual meetings, trainings, or conferences as needed to stay current with best practices in telehealth nursing. • Participate actively in Everlywell's continuous improvement efforts by providing feedback on the platform and service delivery processes when appropriate. • Adhere strictly to all privacy policies and guidel01_STATED
