Requirements

Responsibilities

• Provide high quality care to palliative patients in NJ, PA, GA, VA, OK, KS. • Assess patients’ health by performing physical examinations; obtaining, updating, and studying medical histories and creating the best plan for care. • Effectively communicate with office staff and clinical operations across various internal platforms • Provide high quality, patient centered care to a panel of patients • Collaborate with patients’ entire care team to provide palliative care recommendations and develop holistic, interdisciplinary plans of care • Foster trust and consult with patients, families, and caregivers during visits • Lead sensitive conversations with patients and families regarding goals of care, advance care planning and the transition to hospice care • Order diagnostic testing as medically necessary • Prescribe medications supplies and appropriate medically necessary interventions
