Requirements
• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute. • Eligible for Medicare participation; company bills on your behalf. • 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently. • Comfortable conducting telehealth visits and navigating virtual care platforms. • Thrives in a dynamic, evolving startup environment. • Tech-savvy: Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools. • Spanish preferred. • $120,000 - $140,000 a year
Responsibilities
• Remote patient care: Conduct initial and follow-up telehealth assessments on flagged patients; initiate/adjust plans of care; coordinate with referring and primary care providers—all via secure video/audio platform. • Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits. • Close the loop: Complete documentation/charts within the same day. • Tele-visit management: Efficiently complete 25+ tele visits per day for designated home health patients in the SoCal/OC area, focusing on new admissions, high-acuity flagged patients, and Circadia alerts. • Quality & product feedback: Surface insights to improve virtual workflows and tech integration in a home health setting. • Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams. • How Success is Measured (KPIs) • Visits: Target 25+ patient tele visits/day • Flagged Patients are addressed within 24 hours • Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter. • Adoption & impact: Demonstrated workflow adherence across patient panels; trend toward fewer falls/hospitalizations and improved home health star ratings over time.
