Responsibilities

• Lean on your LPN/LVN clinical background while getting fluent in Solace's proprietary platform, tools, and standards. • Turn empathy and attentive listening into patient relationships built on genuine trust. • Read each patient's situation, sort out what matters most first, and shape a care plan that keeps things moving over time. • Design care plans that go beyond the clinical, factoring in everyday barriers like access to food, getting to appointments, and support in the home.

Benefits
