Requirements
• Experience in patient advocacy, care coordination, social work, healthcare navigation, or a related field. • Deep understanding of Social Determinants of Health (SDOH) and experience working with diverse, often vulnerable patient populations. • Exceptional empathy, active listening, and interpersonal skills, enabling you to build trust quickly and maintain meaningful relationships. • Strong organizational abilities and clear communication, allowing you to manage multiple tasks effectively. • Tech-savvy and capable of quickly learning modern EHR systems, digital collaboration tools, and patient engagement apps. • Action-oriented mindset with a commitment to improving patient outcomes and system efficiency. • Applicants must be based in the United States. • Certification in nursing, patient advocacy, social work, or related field. • Background in community health, health coaching, or experience with Medicare populations. • Fluency in Spanish or other languages.
Responsibilities
• Guide patients through healthcare challenges and clarifying complex medical information in simple terms. • Coordinate seamless care by creating and managing individualized care plans with Careway physicians, addressing social determinants of health such as transportation, food, home support, and digital access. • Proactively identify patient needs, prioritize actions, and coordinate appointments, referrals, and follow-ups. • Empower patients by improving their health literacy and self-management skills through education and coaching. • Collaborate with our clinical and operational teams to share insights for continuous improvement of Careway’s advocacy-driven care model.

To ensure continuity of care for our patients, this position requires a baseline capacity of 20+ hours per week, and we prioritize candidates with 25+ hours of weekly availability.
