Requirements

• Current and unrestricted license as a Registered Nurse in at least one state in the USA, compact license preferred. • 2+ years of experience as a Registered Nurse in a clinic, homecare, ambulatory, hospital or inpatient setting • Experience coordinating care with multiple providers as well as patient support systems. • Ability to manage multiple requests and prioritize accordingly. • Excellent communicator with a high level of adherence to confidentiality. • Successful experience working on a multi-disciplinary care team and a collaborative mindset. • Ability to work well autonomously within the outlined procedures and clinical protocols.

Responsibilities

• Act as a liaison for your own patient panel of medically stable patients. • Monitor and triage lab orders/results, inform patients and families of provider’s instructions, and coordinate care between internal and external providers. • Manage the RN phone line to facilitate care coordination with external providers. • Follow triage protocols, which can include fielding questions from families/patients, etc. • Provide families with condition-specific education and medical management support. • Triage emergent issues in a time-sensitive manner and in alignment with our triage protocols. • Coordinate care with hospitals for medical stabilization admissions to ensure there are no gaps as patients transition back to our care or step up to HLOC.

• Earning Potential Range: $77,000 to $92,000 Annual Base Pay + 15% Annual Performance Based Bonus Potential • Flex PTO policy (3-5 wks/year recommended) + 11 paid company holidays. • Generous Parental Leave. • Competitive Medical, Dental, Vision, Life, and AD&D insurance. • Equip pays for a significant percentage of benefits premiums for individuals and families. • Employee Assistance Program (EAP), a company-paid resource for mental health, legal services, financial support, and more! • 401k Retirement Plan Option. • Student Loan Repayment: • $500/quarter for employees under a year of tenure. • $1,000/quarter for employees over a year of tenure. • Equip reimburses fees associated with initial licensure and license renewals (e.g. application to the state board, exam fees, exam study materials, etc.) for any Equip-required licenses and/or cross-licensure requests. • Providers will be licensed or registered as required in every state where they deliver care. • Continuing Education: • For therapists and medical providers, unlimited online courses through NetCE. • Up to $1,000/year stipend for continuing education. • $50/month stipend added directly to an employee’s paycheck to cover home internet expenses. • Initial work-from-home stipend of up to $500. • Reload of $200 on the home office stipend at the 3-year mark and then every other year after that (year 5, year 7, etc.). • Company Laptop. • Physical Demands • Work is performed 100% from home with no requirement to travel. This is a stationary position that requires the ability to operate standard office equipment and keyboards as well as to talk or hear by telephone.
