Requirements
• Empathetic and clear communicator. You speak with kindness, listen actively, and make others feel at ease – even during difficult or emotional conversations. • Highly organized and detail-oriented. You can juggle multiple patients at different stages in the process and never let things slip through the cracks. • Tech-comfortable. You’re comfortable learning tools like Google Workspace, EHR platforms (e.g., Healthie), or CRMs like HubSpot, and pick up new software quickly. • Mission-aligned. You care deeply about improving care for older adults and are excited to play a key role in someone’s first step toward better mental health. • Experience welcome, not required. You may have worked in healthcare, customer service, or an administrative support role before – but most importantly, you’re organized, compassionate, and eager to grow.
Responsibilities
• Coordinate patient care plans to ensure continuity and quality of care for patients with chronic conditions. • Develop relationships with healthcare providers across different disciplines to facilitate comprehensive treatment approaches. • Monitor the progress of assigned cases, providing regular updates on patient status and adjusting care plans as necessary. • Communicate effectively with patients and their families regarding medical information, treatments, and changes in condition or plan. • Utilize technology platforms for remote monitoring and virtual check-ins to maintain continuity of care outside the traditional healthcare setting. • Analyze patient data and outcomes to identify areas for improvement within the organization's approach to chronic disease management. • Attend regular team meetings, either virtually or in person when required, to discuss case progress and collaborate on complex treatment plans with other care coordinators and healthcare providers.
