Requirements
• Hold a Doctor of Pharmacy (PharmD) • Must be a licensed pharmacist in California • Completion of a PGY2 Ambulatory Care Residency • Completion of a PGY1 Residency and at least 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA) • At least 3 years of ambulatory practice with disease state management • Strong communication, organization, and tech skills • Empathy, integrity, and a team-first mindset • California residents employed by or applying for work at Shields have certain privacy rights. Responsibilities
• Provide comprehensive medication management, including reconciliation, dose optimization, adherence support, and drug‑level assessments aligned with evidence‑based standards. • Partner with physicians, specialists, and interdisciplinary teams to order individualized care plans under a CPA and promote seamless medication coordination. • Monitor therapy effectiveness and safety, identify optimization and intervene as appropriate. • Support quality‑improvement and REMS initiatives. • Provide recommendations to operations by supporting teammates as needed with complex prior authorizations, improving workflows, educating patients, and maintaining ongoing clinical development.
Benefits
• $145,000 and $180,000 • plus bonus • An employee in this position who performs the job duties in any other state can expect a salary rate between $127,000 and $180,000 (Hourly Equivalent: Approximately $61.06 - $85.54 per hour, based on a 40-hour workweek.) plus bonus pursuant to the terms of an any bonus plan if applicable.
