Requirements
• Willing and able to work Monday through Friday 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends • Active, unrestricted RN license with the ability to obtain California licensure within 6 months of hire, plus the ability to obtain additional state licensure as needed • 2 years of people management experience • 5 years in direct care delivery role as a Registered Nurse with at least 2 years of experience in palliative care, hospice care, or complex care management • Strong understanding of cultural competency with the target population • Proven, results-driven leader with experience in change management in rapid-growth environments • Strong organizational and independent working skills • Excellent communication and motivational skills, including conflict resolution techniques • Previous experience working remotely with proficiency in Microsoft and Google applications. Salesforce experience is a plus • CHPN certification
Responsibilities
• Supervise interdisciplinary team members, including Registered Nurses, Licensed Vocational Nurses, Social Workers, Chaplains, and Medical Assistants • Participate in strategic planning to shape the future direction of clinical care delivery • Lead and contribute to key initiatives focused on improving clinical care, quality, and launching new programs • Identify, troubleshoot, and resolve clinical workflow issues that negatively impact care quality or operational outcomes • Ensure adequate and efficient patient care coverage • Support the achievement of KPIs assigned to the care delivery team and perform annual performance reviews • Collaborate with Providers and multidisciplinary professional staff to enhance patient care, implement change and develop process improvements • Provide leadership in the areas of conflict resolution, mentoring, skilled decision making, time management, adaptability and dedication • Provide direct patient care delivery as needed to support organizational goals • Development of and assist in onboarding of newly hired clinical staff (RN, SW, Provider, MA) • Ensure team regulatory compliance (state license renewal, certification check-offs, etc.) • Travel up to 25% of the time may be required for training, meetings, and other business needs • Vaccination Requirement: Employees in patient, client, or customer-facing roles must be vaccinated against influenza. Benefits
• Salary Range: $90,000 - $115,000 annually. Pay for this position will be based on several factors, including, but not limited to: your prior experience and skills related to the position, geographic location, company needs, and current market demands. • California Salary Range: Due to state-specific market demands, the salary range for California is higher and falls between $100,000 - $125,000 annually. • Great humans deserve great benefits!
