Requirements
• Qualified applicants must have at least 1 year of coaching experience or experience working directly with perinatal or perimenopause patients. • Completion of the FamilyWell Perinatal Behavioral Health Coaching Certification program within 3 months of start date (if not previously completed). • Passionate, driven, & inspired by a mission to improve perinatal mental healthcare, particularly in vulnerable patient populations. • Education: • Bachelor’s degree in Social Work, Counseling, Psychology, or a related field (preferred) • Licensure/Certification: • Certified Behavioral Health Coach (through FamilyWell’s Perinatal Behavioral Health Certification Program) OR PMH-C through Postpartum Support International (strongly preferred) • Key Competencies: • Key Competencies: • Empathy and Compassion: Ability to understand and support the unique challenges of pregnant and postpartum patients. • Communication: Strong verbal and written communication skills for effective interactions. • Cultural Sensitivity: Respectful of diverse backgrounds and experiences, particularly in a perinatal context. • Adaptability: We’re a fast growing company, constantly looking to make improvements as we go. Responsibilities
• Support the mental health of pregnant and postpartum patients via telehealth for an assigned patient caseload. • Provide patient education about common perinatal mental health disorders and the available treatment options. • Document patient progress and client encounters in a HIPAA-compliant electronic medical record (EMR) system and client registry provided by FamilyWell. • Communicate patient progress with FamilyWell Care Team members. • Identify patients who are not improving and may need more intensive mental health care and report any concerns to the Care Manager and the Psychiatric Consultant. • Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the program.
